Horizon Care Services – Supported accommodation schemes offer a home with high quality care and support, tailored to individual’s needs. Our residents and their choices are at the centre of everything we do. We support them to be independent, enabling them to take control of their lives, build self-confidence, gain practical and daily living skills, enabling them to progress in their journey to recovery and achieve their aspirations.

Type of schemes:

Our support services are highly flexible and tailored to individual needs, ranging from occasional assistance for a few hours a week to daily support or even intensive interventions over a short period to facilitate recovery goals. We offer a diverse array of schemes designed to address different levels of need. By aligning our support services with individual needs and recovery objectives, we ensure that each individual receives the appropriate level of assistance to thrive in their journey towards independence and well-being.

These schemes include:

  1. Adult Placement Scheme

The Adult Placement Scheme offers dedicated care and support to individuals who require an extended period to progress on their journey to recovery. The duration of placement typically ranges from 6 months to 2 years, tailored to individual needs, level of risks, and life goals. As individuals approach readiness for independent living, we collaborate with relevant Local Authorities/NHS Trusts to facilitate their transition. This process includes assisting with housing list registrations, property searches, and viewing suitable accommodations.

  1. Short-Term Support

Our Short-Term Support program provides stability and intensive support to individuals needing an intense but temporary assistance to recover and integrate into the community. Placements last from 1 to 6 months, depending on individual needs and risk levels. The scheme emphasises time-limited activities aimed at fostering independence, enhancing well-being strategies, and bolstering skills, self-confidence, and resilience to reduce the risk of relapse in the community.

  1. Step-Down Transitional Support

Step-Down Transitional Support caters to individuals ready for discharge from hospitals but necessitating brief yet intensive support to transition successfully into the community. Typically referred by NHS or Local Authorities, individuals in this scheme may face housing or benefits challenges, but must be actively working towards a plan to get an accommodation or a community placement. The duration of community support spans 1 to 6 weeks, focusing on settling post-hospital discharge and minimising relapse risks. Weekly progress monitoring ensures collaborative efforts and a smooth transition to their suitable accommodation.

  1. Outreach Floating Support

Recognising the stress of transitioning to independent living, our Outreach Floating Support offers assistance to individuals who have recently moved from our supported accommodations. This support is time-limited and aims to facilitate community integration. Activities include orientation to the new area, tenancy management tasks, utility bill organisation, setting up the new living space, connecting with local support networks, and promoting active community engagement.

How we support:

Holistic Assessment: We believe in conducting comprehensive holistic assessments in collaboration with individuals. These assessments cover all aspects of their well-being, including physical and mental health, medication, daily living skills, education, employment, accommodation, finances, personal interests, social network, and community support. We also address any risks or behaviours, such as drug and alcohol misuse, and violence and aggression.

Personalised care and support plans: Empowering our residents is central to our approach. We facilitate their involvement in designing personalised care and support plans that align with their needs and life goals. By tailoring these plans, we aim to provide the most effective support, enabling individuals to achieve their aspirations while promoting choice and control.

Tailored interventions: Operating within the framework of the recovery model and strength-based approach, we encourage individuals to recognise and capitalise on their own skills and abilities. Through this approach, we foster self-resilience and empower individuals to maximise their potential.

Regular review meetings: Regular review meetings are integral to our support framework. These meetings allow us to track progress and make any necessary adjustments to the support plans. By celebrating achievements and acknowledging strengths, we motivate individuals to continue working towards their goals.

Collaboration and Involvement: Our commitment to collaboration extends to all stakeholders involved in an individual’s care journey, including family, friends, and relevant professionals. We value their input and actively involve them in the planning and review process. Our accommodation projects

In our accommodation projects, we prioritise creating a high-quality, safe, and homely environment where residents can truly feel at home and maintain their independence.

Our Hatfield project exemplifies this commitment. With five bedrooms located conveniently in the heart of Hatfield, residents benefit from a thoughtfully designed layout. Two ground-floor bedrooms feature en suite facilities, while three additional bedrooms on the first floor boast their own shower rooms. Each room is furnished with essential amenities, including a bed, wardrobe, side table, and chair. The fully equipped kitchen provides designated space for each resident, fostering a sense of ownership and community. Outside, a communal garden offers a tranquil space for relaxation.

Conveniently situated within walking distance of local shops and a bus stop, our Hatfield project boasts excellent transportation links. Residents can easily access St Albans town centre, Hatfield town, 24/7 ASDA, and the University of Hertfordshire via nearby bus routes.
